• Address verification (utility bill, rental contractor, mortgage payment, or property tax statement)
• Immunization record
• Birth certificate (ONLY if student is enrolling for FIRST time in San Diego Unified School District)
• Transcripts (10th-12th grade students only)
• Copy of IEP (if applicable)

• Certificado de nacimiento (SOLO si el estudiante se matricula por primera vez en San Diego Unified School District )
• Transcripciones ( estudiantes de los grados 10 al 12 solamente)
• Copia del IEP (si procede)
